Accident Reports

An accident report is always required if an accident at work or on the way to work results in an inability to work for more than 3 calendar days.

How to create an accident report

A work or commuting injury must be reported immediately to Department of Human Resources and Organization.
Here are the individual steps:

  1. download accident report and fill in on PC (signature not required!).
  2. send notification by e-mail to the Department of Human Resources. The Department of Human Resources and Organization reports the accident to Unfallkasse Hessen (UKH) via the member portal.
  3. the staff member receives confirmation of receipt of the accident report from UKH.

Things to know about the accident report

If an accident at work or on the way to work requires medical treatment, you must first consult a transit doctor. Exceptions: For eye injuries, you may go directly to an ophthalmologist; for ear, nose, throat injuries, you may go directly to an otolaryngologist.


An accident report must always be filed when you have seen a doctor.


Fatal accidents, mass accidents and accidents causing serious damage to health must be reported immediately to the relevant accident insurance institution (telephone, fax, e-mail).
